Assessment / Evaluation Forms
Formative Assessment Report Form – for educators on a one-year plan at the mid-cycle point.
Formative Evaluation Report Form – For educators on two-year Self-Directed Growth Plans at the end of Year One.
Summative Evaluation Report Form – For all educators at the end of the evaluation cycle.
Student feedback collection will be required by BPS starting in the 2015-2016 school year. Student feedback does NOT have to be in the form of a survey. The resources here are just a sampling of what is available for use.
